Normally winch, doors, mirrors, first-aid box, toolbox(or storage box) are my must-haves. The other things to consider depend on your riding style, if you trail much in rough terrain, then a skid plate upgrade is necessary too.

Been to Santo Domingo and it's so much fun just hanging around there! Playa Najayo is lovely too.
